Forms & Information
These forms RTC SUPPLIER INFORMATION FORM and BLANK W-9 FORM are required to be filled out to do business with the RTC (for contracts, POs, address changes, etc.). Send the completed forms to the RTC staff person who asked you to fill out the forms. If you have a contract and/or PO with the RTC and have a name change, in addition to completing the Supplier Information Form and W-9 Form, we will need a copy of your Assignment Letter.
